While we do our best to assist our patients, it is important to know and understand your medical insurance policy. Please take a few moments to review the details of your coverage prior to any visits or procedures. If you are unsure if you can receive coverage while seeing our physicians, or if you are unsure about your network, you may contact your insurance carrier using the phone number listed on your insurance card. We also obtain pre-authorizations from your insurance carrier for most procedures that are eligible for coverage. This assures that you will know exactly what the insurance will pay toward your services and which part you will be responsible for.

Please note we accept assignment for most insurances and bill the claim to the insurance on behalf of the patient. The patient is responsible for any out-of-network co-pays and deductibles and any services rejected by your insurance carrier.
