Eyelid surgery, or blepharoplasty, is one of the top facial cosmetic surgeries for a refreshed, younger-looking appearance. But with a procedure performed in such a visible area, it is normal to be concerned about where your scars are placed and how they will heal.
In this blog, we aim to address your concerns about blepharoplasty scars, guiding you through the surgical process, scar placement, and post-operative care required to see the best possible outcome.
Blepharoplasty scars are hidden in natural folds
Your eyelid lift can be performed all at once on the upper lids, lower lids, or both. The primary goal of the procedure is to remove excess skin and fat that may be contributing to a tired or aged appearance. With skilled surgical techniques, scars are typically well-concealed in either the upper eyelid crease, inside the lower eyelid, or along the lower lash line.
Upper eyelid crease
In upper eyelid blepharoplasty, Dr. Goffas places your incision in the natural crease of the eyelid, which effectively hides the surgical scar. During the procedure, excess skin, muscle, and possibly fat are removed. The incision is then meticulously closed with fine sutures, which are usually removed within a week. With proper care, the scar fades to a fine line as the eyelid heals, usually blending with the color of your skin over time.
Inside the lower eyelid
For lower eyelid blepharoplasty, the approach can vary depending on the specific needs you have. In cases where only fat needs to be removed, a transconjunctival approach may be used. This technique involves making an incision inside the lower eyelid, leaving no visible external scar at all.
If excess skin also needs to be addressed, an incision is carefully placed just below the lower eyelash line. This placement allows the scar to be hidden in the natural shadow of the eyelashes, and over time, the scar fades significantly.
Post-op care helps minimize scarring
Post-operative care is crucial in minimizing scarring after blepharoplasty. Following Dr. Goffas’ instructions will not only promote a smooth recovery, but will also help you achieve the best cosmetic results. Here are some key aspects of post-op care:
- Follow instructions carefully: After surgery, you’ll be given detailed instructions on how to care for your eyes. This will include information on cleaning your eyelids, using prescribed medications, and certain activities to avoid.
- Cold compresses: Applying cold compresses during the first 48 hours post-surgery can help reduce swelling and bruising. It’s essential to do this gently to avoid putting pressure on the surgical area.
- Elevate your head: Keeping your head elevated, including while sleeping, can significantly reduce swelling to help you feel ready to return to social and work activities sooner.
- Avoid straining your eyes: In the initial recovery period, avoid activities that strain your eyes, such as reading, using a computer, or watching television for extended periods. Take breaks to call friends, listen to a podcast, or take a gentle walk to stay mobile without getting too active (Dr. Goffas will let you know when you are cleared for rigorous exercise).
- Avoid sun exposure: After surgery, your eyelids will be more susceptible to sun damage, which can worsen scarring. Wearing sunglasses, hats, and using sunscreen (once approved by Dr. Goffas) can help protect the delicate skin of your eyelids.
- Avoid smoking: Smoking impedes the healing process and should be avoided both before and after surgery.
- Attend follow-up appointments: Regular check-ups with your surgeon are essential to monitor your healing and address any concerns promptly.

My cesarean incision made a terrible and huge keloid scar. I also have a bad scar on my arm from an accident. My 56-year-old arm scar has finally turned white, but my 36 year old cesarean scare is still pink. Can you guarantee me that I will not have a keloid scar from upper or lower eye surgery?
